2. Two (2) passport sized photos
3. Three copies (3) of the Malaysian Identity Cards (for Malaysians only)
4. Three copies (3) of current passport. Persons who do not hold a Malaysian passport should include the endorsements for of the arrivals and departures from Malaysia
5. An introdcution letter from employer (for foreign nationals)
6. Processing fee of RM20.00 in cash or money order payable to the "Akautan Negara Malaysia)
7. self addressed stamped envelop for the delivery of the results
