BAHRAIN

Applicants must provide:
1. Completed Form PS/CID/30 (obtained from the above address)
2. Two (2) passport sized photographs (must be MATTE finished and not glossy) photographs
3. Photocopies of the first four pages of your passport
4. Photocopy of previous residence permits from Bahrain (for non citizens)
5. Full set of fingerprints from your local police authority
6. Processing fee
7. Self-address stamped envelop for the delivery of the results
8. A copy of a letter from the consulate requesting a police clearance may be required

Note: Good Conduct Certificate provided to citizens and persons who held valid residence permits and not to those who held visitor visas.

Relevant document - Police clearance is a culmination of three documents:

Document 1. - Certidao de Antedecentes Criminais (police certificate) - issued by the Federal police. Instruction can be found at: http://www.dpf.gov.br
Document 2. - Certidao de Antecentes Criminias - issued by the State Authority.
Document 3. - Certidao de Distribuicao Civeis e Criminais (Justice Certificate)- issued by the Federal Ministry of Justice. Instructions obtained by contacting the justice department in your city or state
